We are a gastroenterology group with our own in-house path lab. In April 2012, we billed globally for 88305 and 88342 on an esophageal specimen. Recently, this patient's Oncologist requested Her-2/neu testing on the original specimen. Our tech retrieved the specimen from archive, and prepared the specimen for the additional stain. We then sent the specimen to our hospital to be stained (we don't have that stain). They sent the specimen back and our pathologist decided to send the specimen to another lab for FISH study. That lab performed that and is billing that portion separately. Our pathologist then did the final interpretation on the Her-2, and is instructing me to bill an additional 88342.
My questions:
What DOS do I use for the additional 88342? The day the specimen was retrieved from archive? The day our pathologist issued the final interpetation?
I searched the forums, and also found references to using path consult codes, but I don't think that applies here, does it?
