If you bill for two Outpatient CPT codes and one denies for LCD can you appeal this type of denial? I thought if their is not a CPT/DX match and it denies LCD it pretty much was a done deal right? Should a person waste time appealing something of this nature.
 
The first question should be was it coded correctly? The first step should not just be blindly appealing. Was the coding double checked? Was the LCD read and compared to what was billed?
