Wiki Anthem Rejections for diagnosis code

Did I miss something? I've gotten several rejections recently (MA63 Missing/incomplete/invalid principal diagnosis)- but the codes we are using are all active codes. Anyone else having an issue?

Can you post a couple of examples? Payers do often make errors in their denials - their claims processors and system programmers are almost never certified coders and don't always understand the coding rules. At the same time, there may be an issue in your coding that you've overlooked. Without specific examples, it's hard to say which is the case.
