Wiki Anthem Orthotics being denied?

Hi,
I'm having trouble getting my orthotic DME's paid.
I coded it L3000-LT L3000-RT

Anthem is denying it for wrong modifier and deeming it not medically necessary.
What has worked for you ?
Please help this newbie out.

Hi,

I have had some issues with this code as well. From what I understand it really depends on the patients plan and the patients diagnosis alot of the ones I have seen are only covered if the patient is diabetic, have you checked to see if Orthotics are covered under the patients plan?
