Wiki Anterior and posterior osteotomy

My doctor performs anterior and posterior osteotomies during a posterior lumbar fusion. I have always known it as you can only code based on the approach, so I have not been billing the anterior osteotomy but my doctor disagrees. I'm looking for some input and possibly references on this situation.
