When billing antepartum only claims, we have always billed a date span. The from date was the first antepartum visit and the to date was the last antepartum visit with one unit. I am now getting denials from BCBS of TX stating our claim is past the filing limit. The filing limit should begin at the time of the last visit, not the first, correct? Does anyone know where I can find CPT/AMA's guidelines on the official way to bill antepartum only?
