Patient presents for destruction of actinic keratosis and seborrheic keratosis. Patient seen for evaluation two weeks ago. History was taken, examination performed, and treatment options given to patient. Patient chose destruction by liquid nitrogen and we scheduled the procedures for today. There is no change to his history or examination since the last visit. He still desires procedure to be performed.

Procedure: After informed consent was obtained, liquid nitrogen was used to destroy 3 AKs and approximately 15 SKs. A 3mm freeze zone was allowed on each lesion. Verbal instructions were given to the patient on wound care and patient expressed understanding.

To breakdown my question...
Is code 17003 per lesion, or do you report it once for 2-14 lesions after the first (17000)
So if there is 3 do you code

17003 x2

17003 is for 2 through 14 lesions, each according to the description. So I would do it 17000 and 17003 x 2 then use 17111 for the benign lesions.
