Look @ 29897. CPT 29891 it states including drilling of the defect , your op note states debridement down to stable bone base I see no drilling. The removal of loose body and synovitis is included in 29897.

no i would not, because it is included and the ankle is NOT like the knee with separate compartments and your note only states anterolateral portal.
