Hi everyone!
Just had a discussion about using ICD-10 with my manager and a question came up. Since anesthesia really only sees the patient in pre op/surgery/post op, it seems to me that the only 7th character that can be used is "A" (intial encounter) for most circumstances. I am concerned about the payer's denials if the ICD-10 does not match the surgeons selection of this 7th character when processing. What do you think? Thank you!
 
I would assume that that character would refer to the patient's encounter with regards to the specified condition. In other words, it may be the anesthesiologist's first encounter with the patient, but the patient is actually in for sequela. So you would code according to which encounter it is for the patient and therefore not all of them will be initial. I am only a short ways into my ICD-10 online training, but that's the way I understand it so far.
