Wiki Anesthesia codes for ICD-9

I'm confused. I need to know how to code for monitored anesthesia care, ICD-9-cm, a diagnosis of Cyst on knee? They are calling for the CPT and the ICD-9 codes for the anesthesia and CPT and ICD-9 for the reported CRNA.
