Does anyone bill for both the Anesthesiologist and CRNA services during the procedure? Do most insurance companies allow/pay for both?

Some pay for both using 50/50 split, some pay just one claim. If you are contracted with the insurance then it depends on your contract.

Nowadays most companies accept the QK, QX modifiers and pay half for each. The modifiers you choose depend on who/how the cases are being supervised.
It also does depend on what is written in your contracts for the private carriers.
