The list in the front of the ICD-9 book doesn't include several codes we use as first-listed, like V25.2 for sterilization. Claims do get paid with this as the first (or only) code. Does this list apply only to hospital coding?

the list in the front of the book (in the coding guidelines) is the first-only allowed list. There are other V codes that are allowed either first or secondary I assume the one you are using is allowed either first or second.
