doc wants to do a correction of bunion by double osteotomy cpt code with cpt code 28285 with cpt code 28285 -correction of hammert toe,wants to use an allograft, this is asc , with insurance of BLUE CROSS- Would cpt code L8699 be included with procedure ?L8699 is the closest code I came up with ?
