What are your recommendations for the following: A patient has allergy testing (95004 & 95024) spread over 2 dates of service. The test documentation states how many of each were performed on each date. The doc meets with the pt only 1x, after the tests are completed. Can this be recorded in a single progress note and billed on a single DOS (using the date he meets with the pt) or should there be 2 claims, each corresponding to a particular date & the tests done that day?
