Wiki Allergy Shots - Physician required

Is the Physician required to sign off on the chart note created by the Nurse each time she gives an allergy shot or is okay for the nurse to electronically sign off?

Our Nurses have always signed off on their own injection notes. the only notes that are submitted to the physicians for review are the allergy educations and the allergy testing.
