I'm am wondering what code you are using for this drug prior to October 1st for Medicare patient in particular. We are having problems getting payment to even cover the cost of this.
We have used J3590 but are going to try C9399 and wanted to know what others are using. I do not believe the J9010 is correct because this is a different drug than Campath.
 
Your best bet is to contact your insurance companies and find out what they want you to use. I did a brief search and found two that want J3590, one blue cross plan that wants Q9979 and a CMS public meeting agenda from May of 2015 that seeks to establish a new J code for alemtuzumab 1 mg with a preliminary decision to do so. J9010 is for alemtuzumab 10 mg.
