Does anyone have the policy on these codes that specifically states that aetna does not cover these codes if billed more than twice per 1095 days? Is there any truth to this im trying to figure out if i need to appeal this or would it be a waste of time if they really do not cover these codes in within that timeframe pleaseeeeeeeeeee help!!!
