Wondering if you can bill for Advanced Directives such as filling out Health Care Proxy or MOLST forms, if so how do you do it? 2015 CPT states 99497 with dx z02.89 but according to Medicare, they do not list 99497. That is a lot of time not to bill for, suggestions?
