I have recently learned that Medicare will begin paying for coses 99497 and 99487 in 2016. The article I'm seeing says "Advanced care planning including the explanation and discussion of advanced directives such as standard forms (with completion of such forms, when performed), but the physician or other qualified health professionals".

My question is does this mean we have to fill out the forms with the patient? I'm confused!

The way I read the guidelines in the CPT book it says 'with or without completing relevant legal forms.' I don't think any forms are required to be completed. I have a neurologist asking about billing for these services so I am looking into this also.
