When it comes to billing using the 99497 and 99498 CPT codes, does the 30 minute requirement and the additional 30 minutes spent with the patient for advanced care planning have to be completely with the Provider? Or, can the Provider start the discussion and then do a warm hand off to their patient care coordinator to complete the advanced care planning process (and also the remaining time requirement in order to bill using these CPT codes)?
