Our surgon did an inpatient admitmit with a 2 1/2 hour time table with the patient with a decision for major surgery. He is charging a 99223 57 is there something that can be charged as an additional code to show the amount of time spent with the patient since it was extreamly above the normal time period

Yes; look at codes 99356-99357 for Prolonged E/M Services in the Inpatient Setting. Doc will need to document total time, and supporting documentation, of course, but the codes are available. See CPT book for specific guidelines. However, not all payers will pay these codes...
