I have a Medicare denial where an Epogen injection was performed at my clinic and then later, the same MD admitted the patient to the hospital for a different problem (99223). According to CCI, 99223 is bundled into 90772. Since both were performed at different places of service and for completely different problems; what modifier, if any, do I use to attach to 99223 to override the CCI edit? Thanks for any help.
