Wiki adjacent tissue transfer and skin flaps

Hi! For adjacent tissue transfers......I know they require the size of the primary defect (excision of lesion) and secondary defect (donor site for flap) to determine the appropriate code. If the surgeon only states the primary defect size...can we only use that the primary defect size for determining the code? Or would we be able to say that the secondary defect is at least as big as the primary defect in order to repair it?

Would that be the same for autograft or allograft skin flaps? If the surgeon does not state the size of the skin flap.......it would have to be at least the size of the lesion that was excised in order to repair it?
