Wiki Addendum on labs

I have a question on a lab and want to see how other clinics are coding these type of diagnosis.
The pt came in on 10/4 with symptoms of burning, urine urgency. The md did a lab to see if the pt had a uti. The truly of the lab came back on 10/4 the same dos the pt was seen and stated the pt didn’t have a uti, but the md was going to see if the culture grew out bacteria. The dr made an addendum on 10/6 stated pt had bacteria that grew. Would you code the dx of bacteria even if the md stated in the assessment and plan pt didn’t have a uti. The dr addendum the note 2 days later?

I d use dx B96.89 listing some type of bacteria but unknow till get the exact bacteria if listed in lab results. Thus do first dx R30.9 painful urination and B96.89. If types of bacteria supported by lab results look at dx blocks of B95-B98

Are there any ICD-10 guidelines when to and not to code from the labs? Or is there a coding clinic on this?
