It is my understanding that there are no "add-on" codes for time spent for an E/M visit in a nursing home facility. We only have the E/M visit codes with the approximate minutes listed. What happens if the doctor goes over the listed time? It seems we cannot bill for this. Am I correct?

If you look at the 99356 code you will see listed under it the codes it is allowed to be appended to the nursing facility levels are listed there so they can be used. If there is a minimum of 30 minutes beyond the time listed for the visit level documented you can add on the 99356.
