Patient was admited to acute care with a diagnosis of uncontrolled diabetes, then transfered after three days to swing bed.
The Provider dictated a combination transfer and discharge note for both stays. The diagnosis used was 250.00, Type II or unspecified type, not stated as uncontrolled.
Per my supervisor the chart was to be taken back to the provider and told that he needed to change the diagnosis' to uncontrolled diabetes for reimbursement reasons.

My question is; should the provider change the acute stay discharge/transfer diagnosis to uncontrolled diabetes to show the reason for the transfer and continuation of the stay, left the diagnosis of 250.00 for both stays or used 250.02 for both?
