I am curious about everyone's experience when including activity and place of occurrence on 'S' or 'T' code for lacerations or ortho cases. I work at an Urgent Care clinic so we see a significant amount of cases each day and have trained our MA's to include the what, when and where during triage. However, I get a lot of push back and was curious if anyone else is diligently reporting the specifics of how the injury happened or are you using the unspecified Y93.9 and Y92.9 codes.
 
Guidelines state:
Use the full range of external cause codes to completely describe the cause, the intent, the place of occurrence, and if applicable, the activity of the patient at the time of the event, and the patient’s status, for all injuries, and other health conditions due to an external cause.
Do not use place of occurrence code Y92.9 if the place is not stated or is not applicable.
Do not assign Y93.9, Unspecified activity, if the activity is not stated.
 
Noted. I find myself constantly going back to our triage staff to show them what they've missed and the general attitude I get it "Well the patient doesn't want to tell me" or "The patient became argumentative" so I'm wondering if anyone else is having this issue.
