I dug deep and found the policy on extended time for Blue Cross of California: https://www.anthem.com/docs/public/inline/C-08011_CA.pdf

Within that document, there is a link to the accepted dx codes. I downloaded them, fixed a typo in one of the codes, and dropped it into an excel spreadsheet with the descriptions of the codes.

The majority of them are "adverse effect" codes, but there are others, including some for diabetes, hypertension, major depression, etc.
