Wiki about using the acute code if stated in history

If the provider states in HPI problem for 2 weeks
and in the A/P they state Bronchitis

would you code the bronchitis as acute, if not stated acute in the A/P

There must be documentation and actually state acute in the medical record in order to code it as acute.
