99497 ACP vs POLST

Hello - provider submitted 99497, Advanced Care Planning with time documented. I asked her to document what was discussed. She stated that the POLST was completed. Does the POLST support 99497 or is that different documentation and if so, what would that be? I think it's what the patient's wishes are?
