Wiki 99223 hospital code used by multiple providers in one day.

Many times, hospital patients have multiple issues. More than one provider renders their initial visits 99223 to the patients. But insurance carriers deny claim because they will pay only one time who every submits the claim first.
Wondering if anyone else facing similar issues? Is there any suggestion to get paid for the initial services? Could be by using modifier or changing some different procedure code for the services of that day?
 
CMS policy is to allow multiple providers to bill the initial service. Admitting physician with -AI modifier.
Some commercial carriers have a policy to pay only one (the intent being the the admitting) for the initial service and will only reimburse subsequent visits to all other providers. I may not agree with it, but if I have a contract with that carrier, and this is their written policy, the only option I am aware of to receive payment is to bill as 99231-99233.
