Wiki 99211 for depo injections?

In addition to the drug and administration codes, the debate is can we bill a 99211 when a depo injection is performed by the MA without physician interaction?
If no, would the MA consulting with the physician informally suffice the code? Or does the physician actually HAVE to see the patient face to face to support the code?
 
You never bill am office visit 99211 or otherwise for a scheduled injection. If it is not a scheduled injection then you cannot have an MA make the decision to administer the injection .
 
Depo Injection

You cannot bill the Depo, J1050, the injection, and an evaluation and management service if the only reason the patient is coming in is for the Depo injection. If the patient is in the office for another reason you can bill the E&M with modifier 25.
