Can I bill a 99211 w/25 modifier for 96413? The only documetation I have are the vital signs, blood work and order for chemotherapy followed by the infusion notes.

section 60 refers to requirements for incident to services. Based on your information, some of the requirements may not have been met? or not documented for 99211? This might be a useful start. I did not review the notes for chemotherapy infusions. I think that is in chapter 12?

chapter 12 section 30.5 addresses chemotherapy
