Our worker's comp claim has been denied because 99204 is inclusive with 99358. 99358 is paid but not 99204. I have never heard of this kind of payment.

The denial is incorrect and should be appealed. In work comp, 99358 is usually used for record review, and is not included in the evaluation.

isn't 99358 a column 2 to 99202-99205 and 99211-99215? "Code 99358 is a column 2 code for 99204. You may not override the edit." if billed on the same DOS. I'm not sure about worker's comp claims, but that's what the CCI edits show.
