Wiki 99203 and 76882 in same visit

Im getting a handful of medicare denials for an OV (99203) and 76882 during same visit. I filed:
99203 (25)(1,2,3,4)
76882 (1,2,3,4)
(1) G57.61
(2)B35.3
(3)M79.673
(4)R60.0

They paid for the OV but denied the 76882 for "lack of info"

Can anyone help me with this.

First you should link only the medical necessity for the procedure. You would not perform an ultrasound for Dermatophytosis for example. you need to look at why the ultrasound was ordered and use only that diagnosis.
