Does anyone bill this for reviewing a massive chart before seeing the patient, and if you do, does it get paid? Also what do you bill for it? Thanks in advance!
 
No, it would not be payable by most payers since that code for example is a bundled service. The work is is accounted for the in the E&M leveling under MDM.

The code for reporting review of a massive chart before seeing the patient would be prolonged service before or after patient contact 99358-99359 (assuming time is documented and was at least 30 minutes). Some payers will still bundle this code but not all. You can check payer policies online for many payers.
