We are having issues with getting PT and OT claims paid by Medicare when we use the 99 modifier. We have called Medicare multiple times to get guidance on how to put the modifier on the claims. We get different instruction every time and when followed we still received denials. Does anyone have any resources that will help instruct us in how to use the 99 modifier correctly?

While I personally have never needed to use more than 4 modifiers on a line, it seems different MACs have different guidance. I would advise to look for written guidance from your specific MAC.
Noridian states to put 99 on the fourth modifier position. So CPT MOD1, MOD2, MOD3, MOD99. Then line 19 to contain MOD4, MOD5.

At least, that's how I'm reading their guidance even though they both reference the same CMS claims processing manual. 🤷‍♀️
