97 DG guideline confusion

General Multisystem Exam:
"Comprehensive Examination - should include at least nine organ systems or body areas. For each system/area selected, all elements of the examination identified by a bullet should be performed, unless specific directions limit the content of the examination. For each area/system, documentation of at least two elements identified by a bullet is expected." From Medicare Physician Guide

Here's my question.
This states each system selected performed all elements and for each area/system at least 2. So to qualify for a comprehensive exam, at least 9 systems/area must be documented, but how many bullet points - minimum of 2 bullets in 9 system/area, or all bullets in 9 system/area? :confused:

If the exam is a general, multi-system exam then 9 body areas/systems with at least 2 bullets in each system is required. If the exam is a speciality exam, such as cardiology or psych, then all elements in each of the shaded boxes for that specialty must be peformed, with at least one bullet in each non-shaded box. These instructions for specialty exams are what is meant by "...specific directions limit the content of the examination."
