Is 96401 considered an intial code? Example: patient is given Velcade Sub Q (96401) but is given IV infusion of kytril prior to administration - would the IV infusion be coded with 96365 or 96367. Thanks.

96401

96401 is not an initial code and can be billed with any other initial code, ex. 96413 or 96365. I would add the mod to code 96401.
