If a patient that had a previous ablation for a-fib by pulmonary vein isolation, and comes back for another ablation to treat a fib, but pulmonary vein isolation is not repeated, do we then use code 93653 for this ablation rather than 93656 with a 52 modifier?

I would go with 93653. PVI is a key element of 93656. If this is not performed, then 93656 is not the correct code. I would go with 93653 as you suggested since A-fib is a form of SVT.

Now, there was an errata put out in regards to 93656 and use of modifier 52.

"93656 includes each of left atrial pacing/recording, right ventricular pacing/recording, and His bundle recording when clinically indicated. When performance of one or more components is not possible or indicated, document the reason for the reason for not performing."

But be careful here, PVI must still be performed to use 93656. I hope that helps and makes sense.
