Wiki 93228 Inconsistent w/ POS

Has anyone gotten denials for 93228 inconsistent w/ POS? These are remote External mobile cardiovascular telemetry with electrocardiographic recording. Office/11 POS is used but it denials.

Also, how does one of these get paid "remote" reading if the patient is in the hospital (inpatient) and office bills POS 11?

Per CMS, the POS should be where the patient is located at the time the service is performed. If the patient is an inpatient, then the POS should be 21.
