Wiki 92225 Interpretation and Report

Hello,

Would anyone know what "interpretation and report" means in terms of documentation requirements for CPT 92225 - Extended Ophthalmoscopy? Is there an official report format or template that must be used in order to bill this code?

Hello, I'm not a certified coder but from my study I found this, "Ophthalmoscopy, extended, with retinal drawing (eg, for retinal detachment, melanoma), with interpretation and report; initial. The drawing itself is not a report--the report must include findings and impression and a diagnosis justifying the study.

Right, but in terms of the report what is the specific criteria that needs to be met. For example, I believe it has to be separate from the note. Is there a specific format, content etc. Findings and impressions could also be considered interpretation. So there must be a difference...
