I am speaking from a payer viewpoint only, if you are going to submit for payment, the payer will expect that it is billed under the patient with the DX and sufficiently documented in their record. Since you would only be billing one encounter, documenting the parent record is up to your internal guidelines. 90846 signifies that it's family therapy without patient present. I hope that helps.

What are the guidelines for documenting 90846? I can't find much information on what needs to be in the chart note. Thanks for any help!
