I am a payor and want to know if the provider is required to specify what family members (mother, father, caregiver, etc) are present during one of these sessions. I'm finding documentation like "Family able to review goals/tx plan." or "Family reported recent event where csr got in trouble for..." These statements seem vague/generic to me and I feel like the documentation should specify who is in attendance? But I cannot find anything that states this as a requirement?
