When billing for an extended DA that takes 3 sessions to complete, do I use the initial date of service as date of service on the claim form, or would I use the day the report is actually written and completed?

Also, for a basic 90791 is the date of service the date the client was seen or the date the report was actually written?

I am finding differing answers! Please help!
