We bill as a facility and are having troubles getting 87210 paid. As well as not getting payment for 36416.
 
Are you appending modifier QW for the 87210? For the 36416 I would ensure that the code is correct for the type of collection, if it is correct, see if the payor requires a modifier 25 if billed with an E/M.
