Wiki 81002 denials

We've been experiencing an increase in denials with 81002 and an E/M visit on the same date of service. We have been using modifier 25 when appropriate if other services were performed. The denial is stating procedure is incidental to another service. There has been some discussion to add a modifier 59 to the lab but I'm not sure that is the way we should go.

Any ideas??

99212 (25) DX maternal hypertension, other maternal condition, dysuria
76805 DX anatomy scan
81002 DX other maternal condition, dysuria

I know that a dip is part of an OB rountine benefit check but the diagnosis codes are not linking it with a routine issue.
