I work for an independent laboratory that does urine drug testing and we have a question regarding repeat drug screen tests....

Here's the scenario-
A counselor orders a 6 panel drug screen for a patient and the results come back negative... but the counselor has suspicion the patient was on something that day. They want to do another test on the same specimen looking for 6 different drugs... does that scenario allow us to bill 80307 with modifier 91?

Please let me know your thoughts on this, thank you!

Modifier 91 is for repeat on the same day. CPT 80305-80307 is per date of service so if same day its all inclusive.

The treatment centers request the second test on the same specimen typically a few days after they get the results, but according to the CMS guidelines regarding Date of Service- the date of service would still be the original collection since it is under 30 days? Is this correct? Meaning that the original test and the second test on that specimen would have the same date of service... therefore 80307 is all inclusive and we cannot use modifier 91?
