Wiki 8 year post Mohs surgical repair

Hello:

Does anyone know what Dx to use for a complex repair of a 8 year post Mohs surgery that had clear margins? Doctor wants to bill the original 173.21 code. I'm not sure that would be correct since margins were clear and no evidence of recurrence was seen. I'm thinking scar - 709.2.

you cannot use a dx code that does not currently exist for the patient at the time of service. There is no cancer so you can use scar or acquired deformity with the hx of cancer code listed secondary. When a patient has a Mohs surgery, as soon as the surgery is finished the cancer is gone this is why the margins were clear, they do not perform a Mohs unless they know they can eradicate the cancer completely. After a Mohs there is no chemo or radiation necessary.
