Wiki 76 VS 77 MODIFIER

There is a disagreement in our office as to correct use of modifiers 76 and 77. Scenario: Same DOS; same patient, same code......71045-26 w/Dr. Smith; then 71045 with Dr. Jones...71045-26,77; then another 71045 with Dr. Jones.......would it be 71045-26,77 or 71045-26,76. I have always believed the subsequent x-rays with same code and same doctor would get a 77, the same as Dr. Jones' first x-ray, which had a 77 as it was by a different doctor than the first x-ay. Is this making sense? Am I being clear? If not, please let me know...71045-26(Smith).....71045-26,77(Jones).....71045-26,77(Jones).....not 76 on Jones' addtional x-ray after his first got a 77. Dr. Smith...first Doc in whose first was 71045-26....his/her subsequent x-rays w/same code would, of course, get a 76.
